Housing Case Management helps clients who are homeless, in transition or marginally housed search for stable, permanent housing. We provide educational workshops, individual case management services, assistance with housing searches, help with security deposits and linkage with additional resources, as needed.
A housing case manager conducts a comprehensive assessment with each client to learn about their individual needs as well as their financial situation. This includes determining each client’s continuing housing and supportive service needs as well as identifying appropriate housing options and any barriers to securing/maintaining permanent housing.
If you are HIV+ and need help with finding appropriate, permanent housing, get in touch with us and we’ll be happy to meet with you, talk about your situation and decide how to proceed.
